"Target"

नुकताच MBBS झालेला मी दोन्ही RP ना बायपास करून पॅथाॅलाॅजीला थेट Lecturer म्हणून रूजू झालो आणि सुरूवातीलाच सर्वांचा रोष ओढवून घेतला होता. Lecturer ला जेवढी पॅथाॅलाॅजी यायला हवी तेवढी मला नक्कीच येत नव्हती.मला त्याची पुरेपूर जाणीव देखील होती.त्यामुळे मी संध्याकाळ झाली तरी रोज रात्री पर्यंत डिपार्टमेंटला थांबून अभ्यास करू लागलो.

एकदा Sickle cell anemia शिकताना आगरवाल सरांनी माझ्या ज्ञानाचा कस पहाणारा एक प्रश्न विचारला होता. हा "Qualitative disorder" आहे का "Quantitative" ? मी तेव्हा डिपार्टमेंटचा "लिंबू-टिंबू"च होतो. सरांनी "गुगली" टाकलेला आहे हे देखील तेव्हा मला कळले नव्हते.उत्तर आले नाही म्हणून सरांनी चांगलेच फोडून काढले होते.संध्याकाळी मुकाट्याने पुस्तक वाचले व Sickle cell anemia व Thalassaemia यातला फरक समजावून घेतला.Sickle cell anemia मधे RBCs चे "Sickle(विळी,खुरपे,कोयता)" होते ! तर Thalassaemia मधे RBCs चे "Target" होते.

In sports "Target" means an equipment consisting of an object set up for a marksman or archer to aim at.NCC मधे तीन वर्षे काढलेली असल्याने व O.22,0.303 व मशीनगनने अशी "Target" अनेक वेळा भेदलेली होती. त्यामुळे wash out,outer,middle,inner व Bull's eye या "Target" मधल्या सर्व व्याख्या माहित होत्या.त्यामुळे Thalassaemia मधील "Target cells" ओळखणे सहज शक्य झाले.

Numerous target cells are present in patient with hemoglobin E and beta thalassemia trait.Target cells, or codocytes, have an excess of cell membrane relative to cell volume.Macrocytic target cells can be seen in liver disease, and microcytic target cells may be seen in thalassemia.Presence of cells called target cells may be due to:

1.Deficiency of an enzyme called lecithin cholesterol acyl transferase.

2.Abnormal hemoglobin, the protein in RBCs that carries oxygen (hemoglobinopathies)

3.Iron deficiency.

4.Liver disease.

5.Spleen removal.
